Students who currently attend School District 21 do not need to complete these forms. They simply need to complete any forms that are sent via the U.S. Mail from the school or School District as well as forms that come home with students.

These forms are what families new to School District 21 receive when they arrive at the school office to register their child/children. Families are encouraged to register at the school that the child will attend. Parents are welcome to print these forms in advance and complete them prior to coming to school.
